In the absence of widely accepted clinical or paraclinical diagnostic guidelines, a number of recently conducted observational case series have substantially broadened our understanding about the clinical phenotype, diagnosis, and prognosis of ADEM. Epstein-Barr virus (EBV) and Kaposi’s sarcoma-associated herpesvirus (KSHV) are ubiquitous human pathogens that are associated with the development of numerous malignancies. An effective vaccine against HSV has eluded researchers for decades, although a number of strategies show promise (reviewed in [19]). FDC uptake of C3d-Ag complexes via CD21/CD35 promotes survival of B cells within the GC and enhances persistence of Ab secretion and long-term maintenance of memory B cells (5, 15).

The immunobiology of HSV, as it pertains to viral pathogenesis and animal model research, will receive minimal attention in this chapter but will be presented in greater detail in other chapters in this volume. Posttransplant sera were available for 99 of the remaining 110 recipients (4 to 6 months posttransplant). Because of its high toxicity, CMV maintenance therapy was one of the first courses of prophylaxis to be evaluated in this respect. Alternative diagnostic criteria were supportive evidence of CNS disease including seizures or positive findings on electroencephalography (EEG), or focal magnetic resonance imaging (MRI) and/or computer-assisted tomography (CT), with a concurrent positive culture of the skin, eyes, or mucus membranes (SEM). 5 is considered a low positive and may not be positive at all. Bacterial cultures of unroofed lesions were negative, as were both culture and direct fluorescent antibody testing for herpes simplex virus. The world held its breath for fear of a globalized epidemic. Because HSV-1 establishes lifelong latency in neurons and because neuronal cytoskeletal disruption is thought to be the main cause of HSV-1-induced neurodegeneration, understanding the F-actin remodeling pattern by HSV-1 infection and the molecular interactions that facilitate HSV-1 entry into neurons is important. However, infection rates were significantly lower than those reported in the PACTG in the pre-HAART era. Skin and soft tissue infections are most commonly caused by Mycobacterium marinum and Mycobacterium ulcerans, which are both slowly growing mycobacteria. Monitor for adverse gastrointestinal effects with NSAIDs. For example, HSV and HCMV infections in immunocompetent individuals trigger a rapid expansion of natural killer (NK) cells and virus-specific cytotoxic T lymphocytes (CTL) that are important for eliminating infected cells (3, 11, 12). Since their discovery in the 1970s, the HLA genes have been linked to more than 100 diseases, most of which are autoimmune diseases.16,17 Increased susceptibility to infectious diseases has also been positively associated with certain HLA alleles.17 The majority of patients reported to have anti–IFN-γ autoantibodies are of Asian descent, which may suggest the involvement of a common genetic factor.9 However, no linked HLA gene(s) has been identified among these patients to date.

One of the earliest responses to infection is the type I IFN response and the innate pathways modulated by the IFN-inducible, double-stranded RNA–dependent protein kinase R (PKR) system.
